Support to the Health Sector Project (SHSP) – CBHI component SHORT TERM CONSULTANCY FOR CBHI CONTRIBUTION COLLECTION IN LAO PDR ANALYSIS OF ALTERNATIVE OPTIONS AND RECOMMENDATIONS TERMS OF REFERENCES
1. OBJECTIVE OF THE CONSULTANCY The Government of the Lao People’s Democratic Republic has received a grant from the Agence Française de Développement (AFD) to finance the project “Support to the health sector” (SHSP) Swiss Red Cross (SRC) GRET and Lao Consulting Group (LCG) have formed a Consortium in order to offer technical assistance to the Ministry of Health of Lao PDR for the SHSP. A very important part of the Project aims at contributing to the improvement of the financial accessibility to health care services by supporting the Ministry of Health insurance program directed to the informal sector (CBHI). In the context of CBHI extension nationwide, CBHI reform in Vientiane Capital (Lao PDR) and its intended linkage with existing social protection schemes (SSO, SASS, HEF), the objectives of the consultancy are: 1. To identify possible contribution collection mechanisms, in compliance with decentralization and cost-efficiency objectives, for both rural and urban areas. 2. To select two or three favored options, and for those, provide recommendations on implementation mechanism and their costs. 2. TASKS Preparation Review literature on worldwide social health insurance contribution collection mechanisms in the informal sector Make a brief situation review with CBHI Division and documentation Collection and Analysis Collect Information at field level and proceed to analysis. Field work will be realized in Vientiane Capital, including Pakngum District and in Savannaketh Province, including one remote rural district. Report and restitution Produce report and restitute the first results to the MoH

EXPECTED OUTPUTS State of the art in terms of collection mechanisms for health insurance services for the informal sector worldwide; including comprehensive description of successful and failed mechanisms (main features, reasons for success/failure, context); Overview of current issues and bottlenecks of CBHI contribution collection in urban and rural areas of Lao PDR; Identification of alternative solutions for rural and urban areas in Lao PDR: list of alternative solutions - including electricity and water supply billing system, MFI, Banks, mobile phones, Village Union, Lao Women Union, local shops, land taxes – and for each : (i) Comprehensive description of the system’s features, including mapping whenever relevant; (ii) SWOT analysis ; (iii) Impact on management costs (costing); (iv) Conditions to enforce the collection. Among all alternatives solutions, identification of 2 or 3 recommended options; Action plan to implement the recommendations, if accepted; Final report and restitution to the MoH and partners. 4. TIME and PERIOD OF THE MISSION The mission will mobilize a CBHI expert with extensive experience in CBHI management for a duration equivalent to one man month. The one month study would include: - 1 week desk work for documentation/literature review ; - 1 day for a brief review with CBHI Division; - 2 weeks of field analysis in Lao PDR, composed of 3 days of interviews and data collecting per province; - 1 week for the final report. The international consultant will submit a proposal presenting how the work will be conducted. The support of a national expert could be added, fees should be covered by the contracted consultant. The mission is expected to be conducted during the month of July-August 2012. 5. PROFILE REQUIRED International consultant specialized in social heath protection; health financing, public health, CBHI management and CBHI model design, organization, microfinance; 10 years experience in social health protection and CBHI specifically (informal sector); Fluent in English (spoken and written); Professional experiences from Rwanda, India and South East Asia would be an asset. 6.
