Quessionaire for Xbox 360 launch

Hi i m in TY BMS .....i am suppose to launch play station or x box 360..but before that i have 2 conduct a market Research ver in i m suppose to know the feasibility of my product success in a particular target segment(upper class)..

I have 2 prepare 2 Types of questionnaires :-

1. For the Teenagers i.e the gamers &
2. For their parents who would help their Kids purchase the gaming box..

Please help me with how 2 frame Questionnaires for both related 2
Launch of Play station2............& what essentials should i include in my project
